Description of Case
Based on the investigation into a death in the facility. It was discovered that the patient was admitted for treatment of a long standing, poorly controlled seizure disorder. Hospital policy required continuous monitoring of such a patient but said monitoring did not occur due to a known problem with equipment at the nursing station.

Description of Case
Based on the investigation of the care rendered to an infant who presented in the ER bleeding from a tracheotomy site. There were significant delays in diagnosing a bowel which had herniated into the chest cavity. Additionally there were delays recognizing the patient's deteriorating condition and notifying senior staff leading to the patient's death.

NYU Hospitals Center was fined $24,000 in August 2002 based on a survey by the NYS peer review organization's Hospital Compliance Program. They found that residents in the hospital's cardiothoracic surgery program were working in excess of the hours permitted under State regulations.
